Summary

Intermittent rhythmic delta activity (IRDA) on EEG does not reliably distinguish between focal and diffuse brain disturbances. Further analysis of EEG features and clinical data is needed for accurate diagnosis.

Background:

Intermittent rhythmic delta activity (IRDA) is a generalized EEG pattern associated with various cerebral disturbances.
IRDA's non-specific nature makes differentiating focal from diffuse brain abnormalities challenging based solely on this EEG pattern.

Purpose of the Study:

To investigate whether specific electrographic features of IRDA and other EEG findings can differentiate focal from diffuse brain disturbances.
To analyze the correlation between IRDA characteristics, posterior background activity, focal slowing, and underlying brain pathology.

Main Methods:

A blind analysis of 58 EEGs exhibiting IRDA patterns was conducted.
Evaluated electrographic features included posterior background activity, focal slowing, and IRDA characteristics (frequency, distribution, duration, symmetry, abundance).
Clinical diagnoses, consciousness state, and CT findings were retrieved from patient records for comparison.

Main Results:

Of 58 patients, 12 (21%) had focal brain lesions and 46 (79%) had diffuse abnormalities.
Normal consciousness and focal EEG slowing were more common in the focal group, but not statistically significant.
Bilaterally abnormal posterior background activity was significantly more frequent in the diffuse group (67%) compared to the focal group (8%, P<0.0001).
No significant differences were found in IRDA electrographic features between the focal and diffuse groups.

Conclusions:

IRDA morphology alone is insufficient to distinguish between focal and diffuse brain abnormalities.
While posterior background activity can offer clues, IRDA characteristics do not aid in differentiating the etiology of cerebral disturbances.
Further investigation combining EEG findings with clinical data is necessary for accurate diagnosis.
