Background:
- A 57-year-old carpenter presented with a gradually spreading ulcer on his left forearm.
- The ulcer appeared in May 1992 and measured 39 mm x 18 mm.
Observation:
- Histopathology revealed epidermal ulceration, dermal infiltration, central abscess, and granulomatous reactions.
- PAS-positive spores were identified within infiltrating cells and giant cells.
Findings:
- The skin lesion resolved rapidly with oral terbinafine treatment (125 mg/day).
- Immunohistochemical analysis examined cytokine expression before, during, and after treatment.
Implications:
- Terbinafine is effective for treating fungal skin infections.
- Cytokine expression may play a role in the host's response to fungal infections.
