Effect of bulbar acidification on basal secretion of acid and gastrin in dog

Summary

Lowering duodenal pH in dogs significantly reduced stomach acid output, suggesting a role for intrabulbar acidity in regulating interdigestive acid secretion. This challenges existing theories on upper intestinal acid regulation.

Background:

Gastric acid secretion is a complex process regulated by various factors.
The role of the upper intestine, specifically the duodenal bulb, in modulating gastric acid output during interdigestive periods requires further elucidation.

Purpose of the Study:

To investigate the effect of reducing intrabulbar pH on basal gastric acid secretion in dogs.
To explore the potential role of duodenal acidification in the regulation of gastric acid output.

Main Methods:

Surgical creation of isolated mucosal pouches of the stomach and duodenal bulb in dogs.
Perfusion of duodenal pouches with hydrochloric acid (HCl) to lower intrabulbar pH.
Measurement of basal gastric acid output and plasma gastrin concentration.

Main Results:

Acidification of the duodenal bulb rapidly and profoundly reduced basal gastric acid output in dogs.
No significant changes in plasma gastrin concentration were observed following bulbar acidification.
Acidification did not stimulate acid secretion from Pavlov pouches.

Conclusions:

Reduction of intrabulbar pH is a potential mechanism contributing to decreased gastric acid secretion during interdigestive periods.
The physiological significance of upper intestinal mechanisms that stimulate acid secretion in response to reduced intraluminal pH is questioned.
