Summary

Intravenous DSA (diagnostic study angiography) effectively screens for cerebral aneurysms larger than 4 mm. This method shows clinical utility, despite some false positives, for detecting these potentially dangerous vascular malformations.

Background:

Cerebral aneurysms pose a significant risk of rupture and subarachnoid hemorrhage.
Accurate and efficient screening methods are crucial for early detection and management.

Purpose of the Study:

To evaluate the clinical utility of Intravenous DSA (IVDSA) as a screening tool for cerebral aneurysms.
To determine the detection rate and accuracy of IVDSA compared to conventional cerebral angiography.

Main Methods:

IVDSA was performed in 372 cases (390 examinations).
Image quality and diagnostic yield were assessed.
IVDSA findings were compared with conventional cerebral angiography results, particularly in cases with subarachnoid hemorrhage (SAH) or third nerve palsy.

Main Results:

Clinically useful images were obtained in 94.1% of IVDSA examinations.
IVDSA detected 15 aneurysms in 21 cases with SAH or third nerve palsy, missing 4 small aneurysms (≤4 mm).
Aneurysms >4 mm, associated with higher rupture risk, were detectable by IVDSA.
IVDSA showed 33 suspected aneurysms, with 21 confirmed by angiography, resulting in 12 false positives (mainly at anterior communicating and middle cerebral arteries).

Conclusions:

IVDSA is a clinically useful screening method for cerebral aneurysms, particularly those >4 mm.
Further improvements in image resolution and case selection are needed to reduce false positives and enhance diagnostic accuracy.
