Skin reactions to hexylene glycol

Summary

Hexylene glycol (HG) shows lower irritation than propylene glycol (PG) in eczema patients. However, delayed allergic reactions to HG are possible, warranting further investigation in dermatological applications.

Background:

Hexylene glycol (HG) is increasingly used in topical corticosteroids and cosmetics.
Its irritant and sensitizing potential in eczema patients requires evaluation.

Purpose of the Study:

To assess the irritant and sensitizing properties of Hexylene glycol (HG) in patients with eczema.
To compare the effects of HG with propylene glycol (PG) in dermatological testing.

Main Methods:

Patch testing with 50% or 30% HG and 30% PG in water on 823 eczema patients.
Dilution series patch tests and Randomized Open Application Test (ROAT) were performed.
Transepidermal water loss (TEWL) was measured in atopic and healthy skin.

Main Results:

HG caused edema and erythema in 2.8% of patients, compared to 3.8% for PG.
50% HG showed similar visible reactions to 30% PG.
While most reactions were mild, one patient exhibited a positive ROAT to HG, suggesting potential for delayed allergic contact dermatitis.

Conclusions:

Hexylene glycol (HG) appears less irritating than propylene glycol (PG) under occlusion in eczema patients.
Delayed contact allergic reactions to HG are possible and should be considered in clinical settings.
