Bilateral rupture of diaphragm with delayed strangulation

Summary

Background:

A 64-year-old female patient presented with symptoms indicative of shock.
Initial computed tomography (CT) scan revealed a rupture of the left diaphragm with strangulation.

Observation:

Following surgical repair of the diaphragm, the patient experienced herniation of abdominal contents on the right side.
This herniation led to cardiorespiratory collapse, indicating a critical post-operative complication.

Findings:

The right-sided herniation was surgically repaired using Marlex mesh.
Postoperatively, the patient required a partial gastrectomy due to massive duodenal ulcer bleeding.
A dual chamber pacemaker was implanted to manage complete heart block.

Implications:

This case highlights the potential for severe, multi-system complications following diaphragmatic rupture repair.
Management requires a multidisciplinary approach, involving trauma surgery, cardiology, and gastroenterology.
Complex post-operative care is crucial for patients with extensive injuries and comorbidities.
