Summary

Increased respiratory epithelial clearance of 99mTc-DTPA (RC-DTPA) in sarcoidosis patients indicates advanced lung disease and correlates with reduced pulmonary function and elevated serum angiotensin-converting enzyme (SACE). Lymphocytic alveolitis did not significantly differ between groups.

Background:

Sarcoidosis is an inflammatory disease affecting the lungs.
Assessing lung involvement and function is crucial for sarcoidosis management.
Respiratory epithelial clearance (RC) and serum angiotensin-converting enzyme (SACE) are potential biomarkers.

Purpose of the Study:

To investigate the relationship between 99mTc-DTPA aerosol clearance (RC-DTPA) and pulmonary function, SACE, and lymphocytic alveolitis in sarcoidosis patients.
To determine if RC-DTPA can differentiate between stages of pulmonary sarcoidosis.

Main Methods:

Measured RC-DTPA in 49 sarcoidosis patients and 38 controls.
Evaluated pulmonary involvement using chest roentgenograms and pulmonary function tests (PFTs).
Assessed SACE levels and performed bronchoalveolar lavage for lymphocyte counts (Ly%).

Main Results:

Increased RC-DTPA was observed in patients with advanced sarcoidosis (Type II/III) but not in early stages (Type O/I).
Higher RC-DTPA correlated with decreased FVC, TLC, FEV1, Pao2, and DLCO, and increased AaPo2.
Elevated SACE levels were found in patients with increased RC-DTPA and correlated positively with RC-DTPA; Ly% did not differ significantly.

Conclusions:

Elevated RC-DTPA is associated with more severe pulmonary sarcoidosis and impaired lung function.
RC-DTPA and SACE may serve as useful indicators of disease activity and severity in sarcoidosis.
Lymphocytic alveolitis (Ly%) did not appear to be a primary driver of increased RC-DTPA in this cohort.
