Pseudopapilledema associated with abnormally small optic discs

Background:

Pseudopapilledema, or apparent swelling of the optic disc, can mimic serious optic nerve conditions.
Accurate measurement of optic disc size is crucial for differential diagnosis.

Purpose of the Study:

To determine the absolute size of optic discs in individuals with pseudopapilledema.
To compare these measurements with standard values for normal optic nerve heads.

Main Methods:

Littmann's method was used to correct magnification in central fundus photographs.
The absolute area of 35 unselected optic discs with pseudopapilledema was calculated.
Statistical analysis (Student's t-test) was performed to compare disc sizes.

Main Results:

Optic disc area in pseudopapilledema averaged 1.95 +/- 0.33 mm2, significantly smaller (P < 0.001) than normal optic nerve heads (2.73 +/- 0.76 mm2).
No optic disc cupping was observed in the pseudopapilledema group.
High intraobserver (0.045) and interobserver (0.08) reliability was confirmed.

Conclusions:

Markedly reduced optic disc size in pseudopapilledema may be associated with a small optic nerve scleral canal.
This size discrepancy may inhibit intraaxonal flow, leading to secondary swelling and apparent disc prominence.
Pseudopapilledema could be linked to optic nerve head drusen, also associated with smaller optic discs.
