Unilateral lung transplantation for pulmonary fibrosis

Summary

Unilateral lung transplantation offers a successful treatment for end-stage interstitial lung disease, improving lung function and quality of life. This procedure avoids combined heart-lung transplants, allowing donor hearts for other patients.

Background:

End-stage interstitial lung disease (ILD) presents significant challenges for respiratory failure management.
Advances in immunosuppression and surgical techniques have enabled unilateral lung transplantation.

Observation:

Two cases of end-stage respiratory failure due to pulmonary fibrosis refractory to treatment were reported.
Both patients were oxygen-dependent with a poor prognosis.

Findings:

Successful unilateral lung transplantation resulted in significant pulmonary function improvement.
Patients experienced normal arterial oxygen tension and no exercise-induced desaturation post-transplant.
Both patients returned to normal life, with long-term survival observed.

Implications:

Unilateral lung transplantation is a viable option for selected end-stage ILD patients.
This approach avoids the need for combined heart-lung transplantation.
Donor hearts can be preserved for other recipients, optimizing organ utilization.
