Temperature monitoring in digital replantation

Summary

Postoperative monitoring of replanted digits using skin temperature probes can indicate perfusion changes. Temperature drops below 30°C or a significant difference from the control digit suggests poor prognosis.

Replantation and revascularization of digits are complex procedures requiring careful postoperative monitoring.
Early detection of circulatory compromise is crucial for successful digit survival.

Observation:

Skin temperature probes were used to monitor replanted digits, control digits, and ambient temperature in 20 patients.
Clinical assessments included capillary refill, skin color, turgor, arterial Doppler tones, and pulp pressure tracing.

Findings:

Replanted digit temperatures ranged from 26.0°C to 35.0°C, while control digits remained stable (33.0°C to 35.0°C).
Specific temperature patterns indicating poor perfusion included: a >2.5°C drop in the replanted digit with a stable control, the replanted digit dropping below 30.0°C for over an hour, or the control digit dropping below 30.0°C without a clear cause.

Implications:

Skin temperature monitoring provides a valuable, non-invasive method for assessing replanted digit viability.
These findings can help clinicians identify compromised digits early, allowing for timely intervention and improved patient outcomes.
