Summary

This study details two rare cerebral cysticercosis cases with unusual imaging. One involved a giant posterior fossa cyst, potentially the first reported, successfully removed surgically.

Background:

Cerebral cysticercosis diagnosis can be challenging due to atypical presentations.

A 24-year-old patient presented with a massive retroclival posterior fossa cyst mimicking cerebrospinal fluid (CSF) density.
A 44-year-old patient exhibited a progressively enlarging cystic granulomatous mass in the right frontal lobe, associated with seizures.

Findings:

Surgical removal of the giant posterior fossa cyst revealed a racemose cysticercus, confirmed by electron microscopy.
The frontal lobe lesion demonstrated characteristic changes on serial CT and MRI, including wall thickening and perifocal edema.

Implications:

Highlights the importance of considering cysticercosis in differential diagnoses for intracranial cystic lesions, even with atypical imaging.
Suggests that advanced imaging and surgical intervention are crucial for managing complex cerebral cysticercosis cases.
