[A case of giant hepatocellular carcinoma successfully treated by arterial administration of SMANCS]

Background:

• A 68-year-old male presented with a giant hepatic tumor involving the right and left lobes.
• The patient had preserved hepatic function (ICG K = 0.141) despite the large tumor burden.

Observation:

• Characteristic imaging findings and serological markers (positive anti-HCV, high PIVKA II) indicated hepatocellular carcinoma (HCC).
• A metastatic lesion to the right rib was also identified.

Findings:

• Arterial administration of SMANCS resulted in remarkable tumor shrinkage and normalization of tumor markers.
• Combined therapy with SMANCS and radiation controlled the rib metastasis.
• Treatment was well-tolerated with no serious complications, preserving tumor feeders.

Implications:

• Arterial SMANCS administration is a viable therapeutic option for giant HCC with good hepatic reserve.
• This approach offers a potential treatment strategy for advanced liver cancer.
• Further research into SMANCS efficacy in similar cases is warranted.
